Drafting sessions

Session-level table — user, document, mode (whole / insert / edit), model, tokens in/out, cache hits, duration, cost — with group-by per org/user/day.

Landing in Phase 9 alongside Stripe billing. Reads the drafting_sessions table already being populated by every AI call (Phase 3).
